Grants to USA nonprofit, faith-based, community-based, and tribal organizations, agencies, hospitals, and IHEs for programs that help communities reduce falls and fall-related risks among older adults and those with disabilities. Funding is intended to implement, promote access to, and increase sustainability of, evidence-based falls prevention programs.

This funding opportunity has two goals:

  • Goal 1: Through robust partnerships, develop a result-based, comprehensive strategy for reducing falls and falls risk among older adults and adults with disabilities living in your community.
  • Goal 2: Significantly increase the number of older adults and adults with disabilities who participate in evidence-based falls prevention programs, while concurrently pursuing the sustainability of these programs beyond the end of the grant period.

Applicants for this funding opportunity must propose to deliver one or more evidence-based falls prevention programs. These programs must be on the list of pre-approved interventions found in Appendix A of the RFP. Applications proposing to implement programs not included on the pre-approved list will be administratively eliminated.

  • Independent school districts
  • Native American tribal governments (Federally recognized)
  • Native American tribal organizations (other than Federally funded tribal governments)
  • Nonprofits having a 501(c)(3) status with the IRS, other than institutions of higher education
  • Private institutions of higher education
  • Public and State controlled institutions of higher education

Additional Eligibility Criteria:

Domestic public or private non-profit entities including state and local governments, Indian tribal governments and organizations (American Indian/Alaskan Native/Native American), faith-based organizations, community-based organizations, hospitals, and institutions of higher education

To help ensure a wider geographic reach, it is unlikely that more than one applicant per state or tribe will be funded under this funding opportunity. Current or previous ACL Falls Prevention grantees are not excluded from this funding opportunity, but must note previous funding in their application and provide a strong rationale.

Foreign entities are not eligible to compete for or receive awards made under this anouncement.

The deadline for Letters of Intent is December 16, 2019. Applicants are requested, but not required, to submit a letter of intent to apply for this funding opportunity.
